Permission forms
All student athletes must complete the following forms in order to be allowed to participate in interscholastic activities. Please print out these forms and complete prior to the start of the athletic season.
Permission to Transport Athletes - needs to be completed for each athletic season
Physical Form - Needs to be completed by doctor prior to freshmand and junior year for all high school athletes. (must be completed if medical history form contains any positive / "yes" answers)
Medical History Form- Needs to be completed by a parent once a year every year

Parent and Student Notification Steroid Use Agreement - Needs to be completed only once for all seasons.
Student Acknowledgement of Rules- Needs to be completed only once for all seasons.
